Nangangailangan ka ba ng tulong para sa iyong karamdaman?

 

Para sa medical assistance, magpasa ng mga sumusunod na kaukulang dokumento. Ang mga dokumentong ipapasa ay tatanggapin kung ang petsa na nakasaad ay hindi lalampas sa 3 buwan mula sa petsa ng pag-apply:

  • Request letter addressed to Senator Win Gatchalian with contact numbers.
  • Original copy ng Barangay Certificate for Indigence na nakasaad kung bakit ka humihingi ng medical assistance.
  • Certified true copy ng (a) medical abstract, (b) doctor’s prescription for medicines (c) costing for laboratory procedures at (d) iba pang medikal na pangangailangan.
  • Photocopy ng government issued/school ID/birth certificate ng pasyente at ng humihingi ng financial assistance. Authorization letter at/o Special Power of Attorney (SPA) mula sa pasyente kung ang humihingi ay kamag-anak o kakilala ng nangangailangan.

 

Guarantee letters (GL) at/o financial assistance (FA) ay ibibigay lamang base sa mga sumusunod na kaso:

  • Patients who are confined in any health facility.
  • Patients with needs for URGENT medical attention or will undergo operation/procedure such as, but not limited to cancer treatment, blood transfusion, transplant, therapy and others.
  • Patients who received health care management and required for continuous medication and/or laboratory tests.
  • All requests for medical assistance will be forwarded to DOH for the guarantee letter and shall indicate the TYPE of assistance needed:
    • Hospital Bill
    • Medicines
    • Laboratory (2D Echo, CT Scan, MRI, Bone Scan, Ultrasound, Biopsy, Etc.,)
    • Chemotherapy
    • Dialysis Treatment
    • Operation/Surgery/Transplant
    • Blood Transfusion
    • Therapy
    • Others

 

Ang mga request na kumpleto sa kailangang dokumento ay isusubmit tuwing Monday, 9:00 AM to 3:00 PM sa Public Assistance Center (PAC) sa Senate of the Philippines Compound.

Makakatanggap ang pasyente ng notification through text advisory pag ang GL ay puwede nang daanan. Ang release ng GL ay naka-schedule tuwing Thursday mula 2:00 to 5:00 PM.

Patient/claimant must accomplish the constituent information form for constituent database purpose and sign the acknowledgment receipt upon receiving the guarantee letter.

Isang guarantee letter lamang ang ibibigay kada 3 buwan para sa pasyente. Ipinagbabawal ang pag-hingi ng madaming request.

Requests from patients who are undergoing treatment in private/non-government hospitals shall be forwarded to DSWD (NCR, Region 4A, Bulacan, Pampanga) for guarantee letter or financial assistance, subject to their updated guidelines, assessment and approval. Clients must present original documents upon interview at DSWD office.

This policy procedure for the grant of medical assistance through financial support and guarantee letter may be updated without prior notice and shall be approved by the Senator before implementation.
